What You'll Do Provide direct patient home care visits, observes, performs and reports preventative, treatment and rehabilitative nursing procedures while implementing the plan of care Observe and report on the home environment and make recommendation that will enhance patient care, promote safety and prevent adverse effects Work with patient, family and health care team members to coordinate services, promote quality of care and positive patient care outcomes Take verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and documents appropriately Inform Health Care Providers of changes in patient’s condition in accordance with providers orders and documents communications accordingly Provide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort and pain interventions Provide instructions to patient/CG’s and staff in a timely manner Who We're Looking For You are experienced and ready to bring your skills and knowledge to patients who need it most.
