you. The Opportunity Seeking Board Eligible/Certified Family
Medicine physicians. Current CA state license is a plus. The
Practice Adelanto Community Health Center – Adelanto, California
Ontario Health Center – Ontario, California San Bernardino Health
Center – San Bernardino, California As a Federally Qualified Health
Center, this site is eligible for Public Student Loan Forgiveness
(PSLF). Referral resources include WIC, Dental, Psych,
Nutritionist, and a MediCal case worker. Facility hours for this
location are Mon-Fri 8:30am-5pm. Patient mix is a majority adults.
15-18 patients per day per provider. Point of Care testing
available at site; minimal labs. The Community San Bernardino,
