About the Role As a Nurse Practitioner in this full-time, weekday-only role, you’ll serve as a trusted primary care provider for enrolled participants in an innovative program designed to help seniors age safely and comfortably in their communities. You’ll work as part of an interdisciplinary care team, providing direct patient care, collaborating closely with physicians, nurses, therapists, and social workers, and helping shape personalized care plans for each participant. Your typical day might include: - Evaluating participants and documenting findings in their electronic medical record - Performing physical exams, ordering and interpreting diagnostic tests, and providing appropriate treatment - Managing both acute and chronic conditions with a focus on preventive care - Writing prescriptions and developing ongoing care and follow-up plans - Providing education and guidance to participants, caregivers, and staff - Offering compassionate emotional and spiritual support when needed - Participating in team meetings to ensure seamless, participant-centered care What We’re Looking For Required Qualifications - Current Nurse Practitioner license (state-issued) - Minimum 1 year of experience as a practicing Nurse Practitioner - DEA certification with full prescriptive authority - 5 years as a Registered Nurse, with experience in elder care or geriatrics Preferred Qualifications - Bilingual (English/Spanish preferred) - Strong communication skills and a collaborative mindset Why You’ll Love Working Here This organization offers a warm, collaborative environment where every team member plays a vital role in improving quality of life for seniors.
