Job Description:
A company is looking for a California Licensed Behavioral Health
Case Manager.
Key Responsibilities:
Assessment, care planning, and coordination of care for Members
enrolled in the case management program
Maintaining a caseload of members, monitoring reassessment needs,
and facilitating transitions of care settings
Coordinating with interdisciplinary team and maintaining timely and
accurate documentation
Required Qualifications:
CA licensure as LCSW, LMFT, LPCC or RN
Proficient computer skills and understanding of behavioral health
diagnoses
Knowledge and experience in case management, facilities, or with
special populations
Knowledge of DSM V or most current diagnostic edition
Associates degree in Nursing or Masters degree in Behavioral
Health
