Job Description:

The Licensed Vocational Nurse (LVN) assumes responsibility and accountability for the application of the nursing process and the delivery of patient/client care and/or contracted services under the supervision of a registered nurse or licensed designee. The LVN provides service in accordance with Maxim philosophy, standards and policies.Essential Duties and Responsibilities:1. Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or contracted services2. Demonstrates on each assignment the provision of skilled nursing care rendered in accordance with prescribed orders3. Identifies and documents changes in patient's both physiologic and psychosocial condition4. Notifies Director/Supervisor if applicable on the following;a. Physician notification to obtain necessary orders for intervention(s) per state regulationsb. Changes in patient/client condition5. Adheres to Maxim's policies and procedures and applicable protocols6. Identifies and prioritizes actions based upon patient/client care requirements7. Assures patient safety at all times and recipient of services provided8. Verifies that documentation reflects patient/client collaboration, participation in plan of care, expected patient/client outcomes, and selected nursing interventions9. Performs interventions, treatments, and medication administration, according to identified priorities, plan of care, facility policies and patient/client care outcome standards as applicable.Monitors and documents effectiveness of interventions, treatments, and medications10. Evaluates patient/client and provides a safe, comfortable and therapeutic patient/client environment11. Monitors the safety and effectiveness of the environment and equipment including universal precautions12. Participates in activities to facilitate good interpersonal communications13. Communicates observations and significant changes in patient/client status to the Director ofClinical Services and/or Clinical Supervisor /Case Manager or licensed designee14. Documents all physician collaboration/coordination of care in the medical record and communicates this information as needed15. Attends case conferences, team meetings, education in-services as required16. Supports professional practice by observing the following;a. Nurse Practice Actb. Standards of Nursing Practicec. Standards of Patient Cared. Standards of Accrediting Body (ACHC, Joint Commission, etc.)e. State and Local Lawsf. Patient Bill of Rightsg. Maxim Healthcare, Nursing, and Client's Hospital Policies17. Performs other duties as assignedMinimum Requirements:1. Graduate of an accredited vocational nursing program2. Licensed Vocational Nurse in specific state3. Complies with accepted professional standards and practice4. At least one year prior professional nursing experience, preferred5. Willingness to participate in skills advancement training or novice nurse program if applicable6. Current CPR as applicable, for direct patient care7. PPD or chest x-ray if applicable8. Successful completion of new hire training as applicable to job site9. Participation and completion of Maxim's Competency Program10. Strong verbal and written communication skills11. Current Health Certificate (per facility contract or state regulation)12.
